Health Office

Hello, my name is Gina Willocks and I am the school Health Technician. I have a B.S. in Kinesiology from Cal State University Long Beach, with an emphasis in exercise science and over 15 years of experience working as a Health Technician for elementary schools. The health and safety of your child is very important to us here at Allegiance STEAM Academy Thrive. I look forward to working with your family in order to provide a healthy and safe environment for your child. Please inform us of any health problems or concerns you have regarding your child.

Vaccination Policy

Children can not attend school unless an immunization record is presented and immunizations are up-to-date. 

*If your child is unimmunized for medical reasons please notify the health office. You must receive a medical exemption through CAIR-ME

Ready for 7th Grade

Get 2 chickenpox shots and the whooping cough shot if you haven’t had it yet! The Tdap vaccine and 2 doses of chickenpox vaccine are required for all 7TH graders. Your school will need your vaccine records. Talk with your doctor today. 

Health Office Medication Policy

In order to better serve your child it is important that you contact the health office with any health concerns your child has. If your child requires medication while at school whether it is prescription or over the counter your child MUST have a Medication Administration Form signed by a doctor and the parent. This policy is derived from state law and is intended to protect the child’s health by involving the physician in all medication matters. Please see all forms listed below

Child Health and Disability Prevention

California law requires a health examination within 18 months prior to first grade entry. The State law was instituted because children sometimes have serious health problems that would otherwise go untreated. Please contact your child’s physician or community agency to make an appointment for a school entry physical, a Child Health and Disability Prevention (C.H.D.P.), exam.

Required Kindergarten Oral Health Assessment

To make sure your child is ready for school, California law, Education Code Section 49452.8 requires students enrolled in kindergarten in a public school, or in first grade if not previously enrolled in kindergarten, to present proof of having received an oral health assessment within 12 months of initial enrollment. 

**Report of oral assessment is due to the school no later than May 31st of your child’s Kindergarten school year.
